Care Coordination

Care Coordination for Service Users of Solihull Adult Drug Services.

Care Coordination is intended to offer a structured framework for the support of clients with a variety of needs that are supported across multiple agencies.

It helps to ensure continuity of care throughout the clients contact with the treatment system and prevent clients from ‘falling between’ services, and allows individual agencies to offer the best possible service to clients.

The Care Coordinator facilitates communication between agencies and ensures that a there is a systematic and structured review and planning system. The Care Coordinator can be used as a point of contact by the agencies involved to facilitate services for the client across the four tiers of drug provision. The Care Coordinator can also assist the client to gain access to appropriate services whilst minimising the risk of client disengagement from services.

How to access Care Coordination

• A need for Care Coordination may be identified at Triage. Once a comprehensive assessment has been completed the agencies and client would be contacted to discuss and agree that Care Coordination continues.
• A referral can also be made from an agency if they feel that the service user may benefit from Care Coordination.
